What is considered a Full-Time status? Direct Link

  • In order to be considered full-time, partners must average 30+ weekly average hours.

How do I enroll into benefits? Direct Link

When do benefits become effective? Direct Link

Directors and Above: Most benefits become effective on your date of hire. You can enroll in benefits up to 30 days following your date of hire.

All Other Full-time Partners: Most benefits become effective on the first of the month following 30 days from your date of hire. You can enroll in benefits up until the day before your effective date. If your status changes from part time to full time, benefits become effective on the first of the month following 30 days from the date of your change in status.

When enrolling a new dependent, will verification be required? Direct Link

  • For spouses, dependents and domestic partners you will be required to submit dependent verification documents. Documentation must be submitted and receive Alights approval within 30-days of enrollment. Exact details of the required documentation will be provided to you by Alight benefits enrollment portal when you add a new dependent.

Important: If no action is taken to provide proof of verification, and receipt of Alight approval, within the required 30-days of adding dependent(s) to coverage, then coverage for enrolled dependent(s) will drop as of the life event effective date or start of new coverage.

Who is eligible to participate in Carrum?

  • Partners and dependents (18+) enrolled in a Petco UMR-UHC medical plan.

What is the cost for a covered surgery? Petco covers all costs including pre-Op, post-Op, physician fee, facility fee, professional fees, prescriptions and even travel expenses to the surgical center of excellence. Members enrolled in a UMR–UHC plan are required to receive certain procedures—such as bariatric surgery, hip and knee replacements, and spine surgeries exclusively with Carrum.

Who is our pharmacy benefit manager (PBM)?

How do I know which medications are covered under the CapitalRx PBM?

Are GLP-1s covered on the drug formulary?

  • No, GLP-1 medications are not currently a covered drug category. However, alternative medications to GLP-1s are available and covered under the plan.

How do I find out what the cost of my prescription will be?

  • To determine the cost of your medication you can either look up the cost by accessing the CapitalRx Medication Coverage & Pricing Tool or you can call CapitalRx to speak with my pharmacy representative at 833-463-1593.

How do I fill a mail order or specialty prescription?

  • Costco is our mail order pharmacy partner. You can enroll in mail order home delivery by visiting pharmacy.costco.com
  • Specialty medications will be filled by Costco Specialty pharmacy. For assistance call 855-213-0070.

Does Petco offer free virtual telemedicine visits? Direct Link

  • Partners and their dependents enrolled in Petco plans have access to free telemedicine visits.
  • All UMR – UHC participates can schedule for the following care with Teladoc at $0 cost for:
    • Primary360 – for primary virtual care
    • Acute Care
    • Behavioral health
    • Dermatology
  • Kaiser participants have free telemedicine visits but those participating in the Kaiser HSA must first meet the plan deductible.

Does Petco offer mental health benefits? Direct Link

We know that caring for your health goes beyond visits to the doctor, and we offer mental health resources, such as our robust Employee Assistance Program (EAP), free to all Petco Partners regardless of eligibility or plan participation. Through Supporlinc our EAP, partners can access a wide range of tool kits, resources, and services covering topics from mental health to stress management and relationships, offering up to five virtual or in-person counseling sessions per issue, per plan year.

What does Petco match for the Health Savings Accounts for those participating in a UMR-UHC HSA or Kaiser HSA? Direct Link

  • Petco will continue to match contributions to Health Spending Accounts. You must contribute to your health savings account through payroll deduction to receive the match – at least $100 annually if you cover just yourself, or $200 if you cover any dependents. If you contribute after the open enrollment period, your match amount will be prorated. You must complete a one-time ID verification to receive the match.
