Benefit Premiums While on Leave Direct Link
Partners participating in Petco’s health insurance program can continue coverage at the subsidized partner contribution rate for up to 6-months (unless applicable law requires otherwise). Your health benefits must be maintained during any period of leave under the same conditions as if you continued to work. You must continue to pay your portion of benefit premiums while on leave. Upon exhausting a continuous 6-month leave period, the partner ‘s benefits will terminate (unless applicable law requires otherwise) and the partner will be able to elect COBRA. It remains the partner’s duty and responsibility to make sure their portion of the benefit premium is paid, on time, while out on leave.
When taking time away from work or on a leave of absence, you will be billed monthly for any missed premiums (excludes company paid leaves). Benefit premium billing statements will be mailed to your home address as listed in Workday. Payment is due 21 days after the invoice date. Missed payments will be granted a one-time 30-day grace period. Failure to submit payment by the due date, plus grace period, will result in benefits termination effective to the last day benefits were paid through. Therefore, to ensure the continuation of benefits while you on leave, you must ensure payment is received for your portion of the benefits premium.
Summary:
